Child Membership | $39/month
Benefits:
- Two basic cleanings and exams annually
- Fluoride treatment if recommended

Adult Membership | $49/month

Perio Membership
$59/month | 2 annual cleanings
$69/month | 3 annual cleanings
$79/month | 4 annual cleanings

- Two, three, or four annual periodontal cleanings
- Two and exams annually
- Fluoride treatment if recommended

Memberships are not insurance, but a payment arrangement provided by Gary Gibson Dental for services rendered. Membership is not transferable to other dental offices. Payment for any cosmetic or restorative treatment is due at the time of services. Membership dues must be current in order to receive services. A lapse of 28 days in dues will require a reactivation fee of up to six month's dues. Membership club discounts may not be combined with any other offers or discounts, which may include but are not limited to CareCredit purchases. It is solely the patient's responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.
